Since 1990, South Florida Pulmonary Care a Joint Commission Accredited Clinical Respiratory and HME provider has been servicing South Florida patients (Martin to Monroe Counties) requiring Invasive Home Mechanical Ventilation while improving outcomes, patient satisfaction, and reduction of hospital utilization
Every patient placed under our Home Mechanical Ventilation Program must meet Joint Commission and The American Association for Respiratory Care standards for Home Mechanical Ventilation in regard to safety standards, which includes safe discharges from the Hospital or other Post-Acute care facilities. The preferred location for chronic long-term mechanical ventilation is in the home because costs are reduced, quality of life is enhanced, and home mechanical ventilation also reduces exposure to hospital-borne infections and integration with families and the community is maximized.Patient and caregiver satisfaction is greatly improved when they are properly educated and engaged.The indications for both invasive and noninvasive mechanical ventilatory support in the home are increasing as technology and infrastructure support improves. -

REMOTE VENTILATOR MONITORING PROGRAM
REMOTE VENTILATOR MONITORING PROGRAM Mechanical ventilator patients are typically bed- bound and it is difficult to transport them to a Physician’s office for follow-up care. The use of remote ventilator monitoring in the home is rapidly evolving and it allows the clinicians to view and report the patient's ventilation data to the Physicians for review and make ventilator settings changes if necessary in a timely fashion. This innovative technology also decreases exacerbations that may require a hospital visit.

COPD Hospital Avoidance Program
Patient care is at the heart of the South Florida Pulmonary Care COPD Program. All patients referred to SFPC receive a full evaluation by a team of Licensed Respiratory Therapists and include home environment, safety, and infection control. Working with the patient’s PCP and Pulmonologist, SFPC develops a care plan with the Physicians for better outcomes while reducing unnecessary hospital admissions. There is no “one-size-fits-all” care plan at SFPC. We coordinate care with other ancillary contractors to make sure patients receive the support that they need along with patient and caregiver educational initiatives that include disease “anticipatory guidelines” for exacerbation rescue if required. All our programs also include a 24/7 RT Hotline, so patients and caregivers know they have clinical support and triage when needed. A “true hospital-to-home” innovation, this program is also ideal for a post-discharge patient with Pneumonia and other respiratory disorders.

Nebulizer, MDI’s and PDI’s Education:
Most patients don’t receive instructions on the inhaler or nebulizer use
75% of patients don’t know how to use their inhalers, almost all could use a spacer for rescue MDI’s to improve the medication efficacy
Most COPD patients don’t have RESCUE MDI’s 80% of patients don’t use or clean their nebulizer correctly

HIGH FLOW OXYGEN THERAPY (HFOT) PROGRAM
The AIRVO is a humidifier with integrated flow generator that delivers warmed and humidified respiratory gases to spontaneously breathing patients. SFPC provides post-acute patients requiring with an all-inclusive HFOT PROGRAM
Indications for Use
The AIRVO is for the treatment of spontaneously breathing patients who would benefit from receiving high-flow warmed and humidified respiratory gases, including patients who have upper airways bypassed. The oxygen flow may be from 2 - 60 LPM depending on the patient interface, in the home care setting a 10 Liter concentrator is used to provide oxygen enrichment if required.
Indications for Use:
• Hypoxemic Respiratory Failure COPD, Bronchiectasis,
• Hospice- “do-not-intubate patients”
• Eliminate most of the anatomic dead space
• Create a reservoir with high FiO2 in the nasal cavity
• Improve gas exchange
• Significantly reduce the work of breathing
The Home Clinical HFOT Respiratory Therapy Pathway Program is designed to reduce hospital utilization while improving outcomes on patients with COPD, Bronchiectasis, Pulmonary Fibrosis, Pneumonia and Non-Ventilator Tracheostomy patients requiring high flow and humidity. -
Board Certified Pulmonologist
As an option, SFPC can also provide the payor with a Board-Certified Pulmonologist to manage Ventilator patients (Invasive or Non-Invasive) along with clinical management of COPD patients using Telemedical visits with SFPC Licensed Respiratory Therapists. Our Physicians are Board-Certified in Pulmonary, Critical Care Medicine and Sleep Medicine. For ventilator-dependent patients, we can provide our Physicians for in-home mechanical ventilation management and for possible weaning for eventual ventilator liberation.
